Our established complex care service supports Pediatric& adults with Various chronic health issue, such as a neurological condition, or have been impacted by a life-changing accident that requires physical and mental health support, or for those who are living with a life-limiting illness, including end-of-life care, a mental health condition or a learning disability. The care we provide aims to facilitate independence, social interaction, and emotional and mental well-being whilst supporting people to fulfill their potential and live a purposeful and meaningful life.
We are experts in the specialist care of conditions that impact a person’s ability to live well, including children born with various congenital conditions or adults with some neurological conditions like Parkinson’s, multiple sclerosis, or motor neuron disease, and physical disabilities such as muscular dystrophy and cerebral palsy.

We provide RN Case Manager-led clinical interventions, including:
- Respiratory and airway support, such as in-home ventilators care and tracheostomy management;
- Feeding support, including PEG feeding;
- Stoma; and
- Catheter care.
