Skilled Nursing Services

American Care Partners provides skilled nursing services to help patients heal at home and reclaim their independence. When a physician writes an order for homecare services, one of American Care Partners ‘s registered nurses will visit the patient’s home to do an initial assessment. During this comprehensive visit, the nurse will ask the patient questions about his/her discharge instructions, diagnosis, symptoms, medications and more.

 

Plan of Care

The initial assessment helps the nurse to start building the patient’s plan of care. This plan includes details about the home healthcare services that the patient needs, such as:

  • Skilled nursing care needs:
    • Medication set-up and education
    • Wound care/ and Ostomy  Care
    • Physical assessments
    • Disease management education
    • In-home IV therapy administration and teaching
  • Healthcare services needed from additional homecare team members, such as therapy or assistance with activities of daily living
  • The frequency of visits
  • The length of time that the patient will be under the care of American Care Partners` homecare team

Recovering from an illness or a hospital stay may take time. To make sure you are safe, we can send a skilled nurse to assess your needs, review and organize medicines, teach diet guidelines, and identify personal safety risks.

Skilled Nursing is provided by a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) with training and experience in providing care in the home. The nurse communicates frequently with your physician to update your plan of care.

 

Services may include:

  • Evaluation of patient needs
  • Performance of skilled nursing procedures
  • Education for you, your family members and caregivers on disease processes
  • Self-care techniques and prevention strategies
  • Coordination of patient care and services with your physician and other health care team members

Skilled Nursing Goals

  • Increase patient and caregiver knowledge of the disease and steps toward recovery
  • Ensure that the patient and caregiver follow the medication regimen and rehabilitation program
  • Assist hospitals and physicians to safely and efficiently transition each patient to the home or to a community facility
  • Allow patients to recover in the comfort of their homes
  • Decrease the chance that the patient will have to return to the hospital or emergency room
